Intent to Return Form

Introduction

Please use this form to let us know if you plan to return for the following semester. Registration emails for the upcoming semester will only be sent to students who have indicated that they are returning. If your plans change after submitting this form, just let us know by emailing us at registrar@prov.ca.

If you are a graduating student or you do not plan to return for the upcoming semester, you must return your mailbox keys to the mailroom and your fitness centre fob to the Business Office to receive your respective $15 deposits back.

Returning students may also make arrangements to have their assignments and other mail forwarded over the summer. Please go to the Business Office and pay the $15 fee.

If you are a dorm student not returning, contact the Business Office about getting your damage deposit back.
