Student ID Request Form

This form is for our Providence Downtown Students & Seminary Distance Students only. For Otterburne Campus students, please email our Library Staff for assistance. 

Processing your request may take up to 7 business days. When your ID is ready, you will either receive an email that it is ready for pick-up (Downtown Students) or the card by postal mail (Distance Students).
